My clinical practice is located primarily at Brigham and
Women's Hospital where I have a general urology practice and a specialty clinic
one day a week which is devoted solely to male sexual dysfunction. I also
perform surgery one morning a week at Massachusetts General Hospital.

Clinical Program Development
The male sexual dysfunction unit in the Division of Urology,
is directed by me and offers state of the art diagnosis and therapy to men
suffering from erectile dysfunction and other disorders. We have offered
patients the opportunity to participate in clinical trials as a pivotal site in
every new modality of treatment available in the US in the past 5 years
including intracavernosal, intraurethral, and oral therapy one of which (oral
sildenafil) is ongoing, and another (intracavernosal VIP/phentolamine) is about
to commence. We have also recently been the only center in the Northeast to
offer the newest modality of treatment for BPH - transurethral needle ablation
of the prostate (TUNA), and have developed one of the largest series in the
country in this promising few procedure.

Narrative Description of Research
My major focus has been on the development and validation of
hard measures of subjective outcomes in urologic disease. This has resulted in
5 indices that are now widely used throughout the world, having been
linguistically and culturally validated in over a dozen languages. These
include the International Prostate Symptom Score (J Urology 153 Volume
1:99-103,1995), The Symptom Problem Index (Urology 49 Suppl 5A:58-63,1997), the
BPH Impact Index (Med Care 33 Volume 4:145-155,1995), the Brief Male Sexual
Function Inventory (Urology 46 Volume 5:697-706,1995), and the Interstitial
Cystitis Symptom and Problem Indices (Urology 49:Suppl 5A:58-63,1997), work
which has been funded by an NIH RO-1 grant. Future work, for which I have
applied for another NIH RO-1 grant, will involve the study of prostatitis
syndromes and includes the development and validation of a symptom index - a
key component of a proposed 5 year multi-center study. I have also been an
investigator in clinical trials for new modalities of therapy for erectile
dysfunction, some of which are now standard treatment (NEJM 334 Volume
14:873-877, 1996). Two other treatment trials are currently underway.
